COMPLETE YOUR REGISTRATION:
After contacting MLMS, you will receive the following forms for enrollment. Please sign and return.
❖ Complete Registration Form/ including $50.00 Registration Fee
❖ Receive Agreement Letter from MLMS
❖ Review and understand Program Policy & Procedure
❖ Complete and return Emergency Consent Form
❖ Complete and return Child Health Report (Signature from doctor dated after September 1st, 2018 is required).
